Community Collaborations

HWFPC has sought innovative approaches toward improving community health which have significantly included partnering with other organizations and agencies already serving Pike County. A way we can fulfill our mission is to serve as Fiscal Sponsor to other not-for-profit programs also seeking to provide impactful health improvements. We currently function as Fiscal Sponsor to numerous charitable initiatives, including Pike County CEO, Pikeland Snack Pack, and Griggsville-Perry Snack Pack Programs. In 2012, while serving as Fiscal Sponsor for the Pittsfield Skate Park Program, that project won an Illinois Governor’s Hometown Award for helping in the development of Pittsfield’s King Park Skate Park.

The Foundation was a founding member of Pike County Community Health Partnership (PCCHP) and served as its Fiscal Sponsor. PCCHP was a collaboration of Pike County’s significant healthcare providers including Illini Community Hospital, Pike County Health Department, Mental Health Centers of Western Illinois, and other organizations including Two Rivers, RC&D who committed to collaborating to strengthen the health and wellness of Pike County. Among the projects, they were able to conduct include: conducting the Pike County Health Needs Assessment, creating a Food Pantry Community Garden, providing Ruby Payne Culture of Poverty Training, providing Fitness/Interval Stations at Pittsfield’s Lowry Park, and offering the Illini Fitness Wellness Program. PCCHP was recognized for its work including being awarded a Network Development Federal Grant by the Health Resources Service Admin. (HRSA) Network; recognized by the National Rural Health Assoc. (NRHA) as “Outstanding Rural Health Partnership” and receiving an Award of Merit from Illinois Rural Health Assoc. (IRHA).

Since our founding, we have participated on various boards of organizations that provide services and programs to create sustainable change and impactful health improvements, including:
